Chunk, embed, and index a document into the workspace (dense + lexical lanes). Re-ingesting the same externalId replaces the prior version. Optional permissionSet gates who can retrieve it.

Input parameters:

Hybrid (dense + lexical) retrieval over a workspace, filtered by the caller’s permissions. Returns the top matching chunks.

- `permissions` (array): Caller permissions; only chunks whose permissionSet intersects are returned.
- `query` (string, required): Natural-language query.

Retrieve permitted chunks and compose a grounded answer with [n] citations. Refuses to use outside knowledge.
